Please complete this form by March 1st to confirm your child's return to FMA for the 2023/2024 Academic year. The fee of $200 for school supplies is due no later than May 1st by cheque, cash or EMT. If this form is not returned by March 1st, your child's placement may not be secure as we will accept new students from our wait list. Please take a look at our new payment schedule for next year.
